The anterior sacroiliac ligaments are the weakest component of the SI ligament complex, failing first in pelvic ring injuries from external rotation forces. They are thinner and less substantial than the posterior SI ligaments, reflecting the predominantly posterior loading of the SI joint under body weight.

Clinical Notes

Anterior SI ligament disruption contributes to the widening of the SI joint visible on AP pelvis radiograph in external rotation pelvic ring injuries. Internal fixation of the symphysis controls the anterior ring, while the posterior ring is stabilised separately if the posterior ligaments are disrupted.
