Anterior Radioulnar Ligament of the DRUJ

The anterior radioulnar ligament of the distal radioulnar joint is the palmar component of the DRUJ capsule, running from the palmar rim of the sigmoid notch to the palmar ulnar styloid base and ulnar fovea. It resists dorsal ulnar head subluxation during pronation and is the primary stabiliser of the DRUJ in full pronation.

Resists dorsal subluxation of the ulnar head during pronation and contributes to DRUJ stability throughout the pronation-supination arc.

Clinical Relevance

Clinical Notes

The anterior DRUJ ligament is disrupted in distal radius fractures with volar displacement (Smith fractures) and in isolated DRUJ volar instability. MRI arthrography demonstrates the ligament with the forearm in supination. Repair or reconstruction is required in chronic volar DRUJ instability. It forms the palmar component of the TFCC structural ring around the ulnar head.

DRUJ Volar Instability

Anterior DRUJ ligament tear allowing dorsal ulnar head subluxation during pronation, producing a painful clunk at the wrist in pronation managed by ligament repair or DRUJ reconstruction.
