The iliotibial ligament — a condensation of the anterior inferior iliac spine (AIIS) periosteal attachments connecting the AIIS to the hip capsule and rectus femoris straight head origin.

Provides the periosteal attachment for the straight head of rectus femoris and the anterior capsular attachment to the AIIS.

Clinical Relevance

Clinical Notes

AIIS impingement (extra-articular FAI) occurs when a prominent AIIS from a previous avulsion fracture impinges against the femoral neck during hip flexion, producing anterior hip pain. Arthroscopic or open AIIS reduction osteoplasty addresses the impingement.

Prominent AIIS from healed apophyseal avulsion or developmental prominence impinging against the femoral neck during hip flexion, producing anterior hip pain in young athletes managed by arthroscopic AIIS trimming.
