The anterior (palmar) wrist capsule reinforces the radiocarpal and midcarpal joints anteriorly, blending with the intrinsic palmar ligaments. It is tightest during wrist extension and provides restraint against excessive dorsiflexion.

Clinical Relevance

Clinical Notes

The anterior wrist capsule is disrupted in Barton's fracture (volar shear fracture of the distal radius) where the capsular attachment avulses the volar lip fragment. Volar plate fixation for Barton's fractures restores the capsular attachment. In wrist arthroscopy, the palmar capsule is visualised from the 3-4 portal and the capsular insertions of the intrinsic ligaments are assessed.

Volar shear fracture of the distal radial articular lip avulsing the anterior wrist capsule attachment, producing radiocarpal joint subluxation requiring volar plate fixation to restore capsular integrity.
