The ankle mortise capsule is the fibrous sleeve enclosing the tibiotalar joint, reinforced anteriorly and posteriorly by capsular thickenings and laterally and medially by the ankle ligament complexes. The anterior capsule is the thinnest region.

Function & Actions

Contains the ankle joint, allows the 50-degree plantarflexion-dorsiflexion range, is reinforced by the deltoid and lateral ligament complexes, and is the capsule that undergoes anterior capsular impingement (ankle impingement syndrome).

Clinical Notes

Anterior ankle capsular impingement from capsular hypertrophy or osteophytic spurs produces anterior ankle pain on dorsiflexion — common in soccer players and dancers. Arthroscopic anterior capsulectomy and osteophyte removal restores dorsiflexion range.
