A bursal space between the flexor retinaculum and the underlying flexor tendons as they enter the carpal tunnel, separate from the individual flexor tendon sheaths. It may become distended in carpal tunnel syndrome and in flexor tenosynovitis.

The flexor retinaculum bursa space is the anatomical target of hydrodissection techniques for carpal tunnel syndrome, where high-volume injection between the retinaculum and flexor tendons releases retinacular adhesions and reduces compressive forces on the median nerve. Ultrasound-guided hydrodissection targets this periretinacular space for non-surgical carpal tunnel treatment.
