Friction bursitis between the tibialis posterior tendon and a prominent accessory navicular produces medial midfoot pain and tenderness directly over the navicular tuberosity, exacerbated by footwear pressure and managed with orthotics, padding, and surgical ossicle excision when conservative management fails.
