Thumb MCP Joint Palmar Bursa

bursa articulationis metacarpophalangeae pollicis

A small bursa at the palmar aspect of the thumb metacarpophalangeal joint, between the volar plate and the flexor pollicis longus tendon. It reduces friction as the FPL tendon passes over the MCP joint during thumb flexion.

Clinical Notes

Thumb MCP joint palmar bursitis contributes to palmar thumb pain in trigger thumb syndrome and FPL tendinopathy. It is distinct from the FPL radial bursa at the wrist level. Volar plate injuries from hyperextension sprains (gamekeeper's injury pattern) may involve this bursa. Ultrasound identifies palmar MCP fluid collection distinguishing bursitis from joint synovitis.

Bursal inflammation at the palmar thumb MCP joint from FPL tendinopathy or volar plate injury producing palmar thumb base pain with active flexion, managed with splinting and targeted injection.
