A bursa between the tensor fascia lata muscle belly and the underlying greater trochanter or iliac crest, reducing friction as the TFL contracts and slides over the lateral hip. It may become inflamed in greater trochanteric pain syndrome and in iliotibial band syndrome at the hip level.
TFL bursitis contributes to the lateral hip pain complex (greater trochanteric pain syndrome), often coexisting with gluteal tendinopathy and trochanteric bursitis. Ultrasound distinguishes TFL bursa inflammation from the trochanteric bursae proper. Runners and cyclists with tight iliotibial bands develop TFL bursitis from repetitive compression of the TFL against the hip. Injection at the TFL-greater trochanter interface combined with ITB stretching addresses this component.
