A potential bursal space along the spigelian line (linea semilunaris) at the lateral border of the rectus abdominis, where Spigelian hernias protrude between the transversus abdominis and internal oblique layers. The bursa develops from fascial friction at this junction.

Clinical Notes

Spigelian bursitis contributes to lateral abdominal pain at the rectus border in the absence of a fascial defect, and must be distinguished from Spigelian hernia (with defect) by ultrasound. Both produce pain at the spigelian zone but only the hernia shows a fascial gap. Spigelian hernias are typically interparietal — between the muscle layers — and may be missed on clinical examination.
