A bursa at the mid-belly of the semimembranosus muscle at the posterior medial thigh, at the muscle-tendon junction zone subject to compressive and shear forces during hamstring loading. It develops in runners and cyclists from repetitive posterior compartment loading.

Mid-belly semimembranosus bursitis produces posterior medial thigh pain distinct from proximal hamstring origin bursitis at the ischial tuberosity. The pain is located at mid-thigh level and is reproduced by resisted knee flexion and passive hip flexion-knee extension stretching. Ultrasound identifies the bursal fluid collection at the muscle-tendon transition zone. Progressive eccentric loading rehabilitation targeting the hamstring complex is first-line, with ultrasound-guided injection reserved for resistant cases.
