The semimembranosus has its own bursa separate from the semimembranosus-gastrocnemius bursa that forms Baker cysts. This isolated semimembranosus bursa reduces tendon-bone friction at the posteromedial tibial condyle. Its enlargement produces a posteromedial knee swelling distinct from the typical posterior Baker cyst.

Isolated semimembranosus bursitis is distinguished from a Baker cyst by its posteromedial rather than posterior popliteal location. Ultrasound identifies the bursa between the semimembranosus tendon and the tibia. It is managed with aspiration and injection when symptomatic.
