Sacrococcygeal Bursa

A bursa at the sacrococcygeal joint between the apex of the sacrum and the first coccygeal segment, reducing friction at this secondary cartilaginous joint during coccygeal movement. It may develop following direct trauma to the coccyx and contributes to coccydynia.

Sacrococcygeal bursitis is a recognised cause of coccydynia in patients with a history of direct coccygeal trauma, prolonged sitting on hard surfaces, or childbirth injury. MRI identifies bursal fluid and bone marrow oedema at the sacrococcygeal junction. Ultrasound-guided corticosteroid injection at this level provides both diagnostic confirmation and therapeutic relief in up to 70% of patients. Coccygectomy is reserved for refractory cases.

Persistent tailbone pain from bursal inflammation after a fall onto the coccyx, producing pain with sitting, standing from seated, and defecation, managed with cushioning, NSAIDs, and injection.
