A small bursa at or adjacent to the sacral hiatus between the sacrococcygeal ligament and the underlying sacral bone, developing from repetitive pressure on the sacral tip in coccydynia and from caudal injection procedures.
Sacral hiatus bursitis contributes to coccydynia and lower sacral pain in patients who sit for prolonged periods or have had multiple caudal epidural injections. Ultrasound-guided injection at the sacral hiatus level addresses this bursa. Clinically it is indistinguishable from sacrococcygeal joint bursitis without imaging.
