The iliopsoas bursa (psoas bursa) communicates with the hip joint in 15% of normal individuals and in up to 35% of patients with hip OA or RA. This communication can produce a large anterior hip mass from joint fluid decompressing into the bursa (burseal distension from hip effusion). The bursa may compress the femoral nerve producing anterior thigh paraesthesia in large bursae from inflammatory arthritis.

Snapping hip syndrome (coxa saltans interna) from the iliopsoas tendon snapping over the iliopectineal eminence occurs within the psoas bursa — the snap is produced by the tendon moving from medial to lateral over the bony eminence. Ultrasound-guided iliopsoas tendon injection within the bursa provides diagnostic (dynamic snap reduces) and therapeutic relief.
