A bursa in the preauricular region between the skin and the underlying temporomandibular joint capsule or parotid fascia. It may develop from chronic jaw clenching, direct pressure from headsets, or repetitive mandibular movement in athletes who use mouthguards.
Pre-auricular bursitis must be distinguished from TMJ synovitis, parotid gland pathology, and preauricular lymphadenopathy. Ultrasound identifies a thin-walled cystic structure anterior to the tragus. It is occasionally seen in violinists and mandibular sports athletes. Aspiration is curative in most cases.
Bursal inflammation anterior to the ear producing a soft preauricular swelling that mimics a parotid lesion or TMJ effusion, confirmed as a bursal structure by ultrasound.
