A small bursa between the ligamentum flavum and the overlying interspinous ligament in the posterior cervical spine, forming in response to repetitive compressive loading of the ligamentum flavum during cervical extension.

Clinical Notes

Ligamentum flavum cysts in the cervical spine are an uncommon but recognised cause of cervical myelopathy and radiculopathy, arising from the posterior ligamentous complex. They project into the posterior spinal canal and are identified on MRI. Surgical excision during cervical laminectomy provides definitive treatment. They are associated with cervical facet joint arthrosis and ligamentum flavum hypertrophy.

Distension of the posterior cervical bursa or ligamentum flavum synovial cyst compressing the cervical spinal cord, producing myelopathy with hand clumsiness and gait disturbance, requiring surgical decompression.
