A small bursa between the posterior elbow capsule and the overlying triceps tendon, reducing friction during elbow extension when the triceps compresses against the posterior capsule and olecranon fossa. It may be identified on MRI as a small fluid collection in the posterior elbow recess region.
Posterior elbow capsule bursitis contributes to posterior elbow pain in throwing athletes and weightlifters, where repetitive forceful extension compresses the posterior capsule. It must be distinguished from olecranon bursitis (more superficial) and posterior elbow impingement (intra-articular). Ultrasound differentiates this deep bursa from the superficial olecranon bursa.
Deep posterior elbow pain from bursal inflammation between the triceps and posterior capsule, exacerbated by resisted elbow extension and distinguished from olecranon bursitis by its deep, non-fluctuant nature.
