A small bursa between the anterior surface of the piriformis muscle at its sacral origin and the anterior sacrum, reducing friction during piriformis contraction against the pelvic surface. It may become inflamed in piriformis syndrome and sacroiliac joint dysfunction.

Piriformis origin bursitis contributes to deep pelvic and posterior hip pain in piriformis syndrome, compounding the neural compression component. MRI demonstrates fluid at the anterior sacral piriformis attachment in symptomatic patients. Ultrasound-guided injection targeting the piriformis origin provides both diagnostic and therapeutic value. It is distinguished from sacroiliac joint pain by its more lateral and inferior location.
