Peroneus Tertius Ankle Bursa

A small bursa around the peroneus tertius tendon in the fourth extensor compartment at the ankle, alongside the EDL. The peroneus tertius assists ankle dorsiflexion and eversion, with its bursa sharing the lateral fourth compartment space.

Clinical Notes

Peroneus tertius bursitis is uncommon but contributes to anterolateral ankle pain in individuals where the tendon is well-developed. In the 13% of individuals where the peroneus tertius is absent, this bursa does not exist. Its presence is confirmed by identifying the tendon inserting on the dorsal fifth metatarsal base on MRI or ultrasound.

Peritendinous inflammation around the peroneus tertius at the ankle producing anterolateral foot pain with dorsiflexion-eversion activities, managed with injection into the lateral fourth compartment.
