A bursa between the peroneal longus tendon and the cuboid bone as the tendon changes direction at the cuboid notch to cross to its insertion on the first metatarsal base. This acute directional change creates significant friction requiring bursal lubrication.
Peroneal longus bursitis at the cuboid notch produces lateral midfoot pain in dancers and gymnasts who load the lateral foot in plantarflexion. Peroneal longus tendon split tears and os peroneum fractures occur at this same location. Ultrasound identifies bursal fluid adjacent to the peroneal longus as it wraps around the cuboid.
Bursal inflammation at the cuboid notch producing lateral midfoot pain with weight-bearing, associated with os peroneum pain syndrome and peroneal longus split tear, managed with orthotics and targeted injection.
