A small bursa between the peroneal tendons and the posterior fibula at the level of the fibular groove, reducing friction as the tendons change direction around the lateral malleolus. It is distinct from the retromalleolar bursa at ankle level.
Peroneal groove bursitis presents as lateral lower leg pain and swelling just proximal to the lateral malleolus. It is associated with peroneal tendon subluxation and superior peroneal retinaculum tears. Ultrasound identifies bursal thickening and associated tendon pathology.
