Orbital Periosteal Bursa

A potential bursal space between the periorbita (periosteum lining the bony orbit) and the orbital walls, allowing the periorbita to slide slightly during eye movement and orbital volume changes. It may develop a true fluid collection in inflammatory orbital disease and orbital trauma.

Clinical Notes

Subperiosteal abscess of the orbit (the most common complication of ethmoid sinusitis in children) develops in this space between the medial orbital wall periosteum and the bone. CT demonstrates a lenticular subperiosteal fluid collection displacing the periorbita medially. Surgical drainage via an external or endoscopic approach accesses this periosteal bursal space. Post-traumatic subperiosteal haematoma also occupies this space.

Subperiosteal Orbital Abscess

Bacterial infection tracking from the ethmoid sinuses through the thin lamina papyracea into the subperiosteal orbital space, producing proptosis, restricted eye movement, and fever requiring urgent surgical drainage.
