A potential bursal space between the lacrimal sac and the periosteum of the lacrimal fossa on the medial orbital wall. The lacrimal sac occupies the lacrimal fossa and a thin periosteal layer separates it from the lacrimal bone and frontal process of the maxilla.
The lacrimal sac periosteal space is relevant in dacryocystorhinostomy (DCR), where the sac must be elevated from the lacrimal fossa periosteum to create the bony ostium connecting the sac to the nasal cavity. Lacrimal sac mucocele expands into this periosteal space. Dacryocystitis produces inflammation and abscess in this region, occasionally extending into the orbital periosteal space.
Acute dacryocystitis spreading into the periosteal lacrimal fossa space producing a tender medial canthal swelling with skin erythema and pointing, requiring incision and drainage followed by DCR.
