A small bursa between the medial and lateral malleoli at the ankle mortise level, located anterior to the distal tibiofibular joint. It may develop from repetitive pressure between the malleoli and external surfaces during kneeling or from tight footwear compressing the ankle.
Intermalleolar bursitis presents as a localised swelling at the anterior ankle between the malleoli, worsened by tight shoe wear and kneeling activities. It is distinguished from ankle joint effusion by its location anterior to the joint line and its superficial nature on ultrasound. Padding and footwear modification are usually sufficient treatment.
