A small bursal space between adjacent lumbar laminae covered by the ligamentum flavum, present in the interlaminar window. It contains epidural fat and epidural vessels and is the space targeted in interlaminar epidural injections.
The interlaminar window is the entry point for interlaminar epidural steroid injections, where the needle passes through the interspinous ligament and ligamentum flavum into the epidural space. Loss of resistance technique identifies the transition from ligament to epidural space at this bursal level. The interlaminar approach is the standard for lumbar epidural anaesthesia and is less technically demanding than transforaminal approaches.
Complications of interlaminar epidural injection including inadvertent dural puncture, intravascular injection into epidural vessels, and epidural haematoma from anticoagulated patients, managed by recognition and appropriate intervention.
