Inflammation of the intercostal groove bursa contributes to intercostal neuralgia patterns that are exacerbated by deep breathing. It must be distinguished from rib metastases, pleural pathology, and herpes zoster. Intercostal nerve blocks target the neurovascular groove directly inferior to the rib angle to reach this space. Rib periostitis from trauma or radiation may involve these bursae.
