The iliopsoas bursa is the largest bursa in the body, communicating with the hip joint in 15% of normal individuals through a posterior capsular opening. This communication means that hip joint effusions can decompress into the iliopsoas bursa producing a palpable anterior hip mass. In rheumatoid arthritis, infected joint effusions can decompress into the iliopsoas bursa producing a psoas abscess-like presentation.

Clinical Notes

Iliopsoas bursitis produces anterior hip and groin pain worsened by hip flexion, reproduced by resisted hip flexion. Ultrasound demonstrates the distended bursa between the iliopsoas tendon and the anterior hip capsule. Communication with the hip joint (present in 15%) means that hip joint pathology (OA, inflammatory arthritis) can produce concomitant iliopsoas bursitis requiring joint-directed treatment alongside bursal injection.
