A bursa located between the iliolumbar ligament and the adjacent iliac crest, reducing friction at the broad ligamentous attachment. It can become inflamed from repetitive trunk lateral flexion and rotation loading.

Clinical Relevance

Clinical Notes

Iliolumbar bursitis contributes to posterolateral low back pain at the level of the iliac crest, often mistaken for iliac crest contusion or superior cluneal nerve entrapment. Tenderness localises just above the posterior iliac crest. Ultrasound-guided injection at the iliolumbar ligament attachment is both diagnostic and therapeutic.
