A potential bursal space between the iliacus muscle and the iliac fascia in the iliac fossa, forming part of the iliopsoas compartment. It may accumulate fluid in iliacus haematoma, iliacus abscess, and in conditions causing retroperitoneal fluid collections.
The iliacus fascia bursal space is the site of iliacus haematoma in anticoagulated patients, producing femoral nerve compression with anterior thigh numbness and quadriceps weakness. Spontaneous iliacus haematoma is a recognised complication of anticoagulation therapy. CT identifies the haematoma confined within the iliac fossa by the iliac fascia. Haematoma evacuation is indicated for progressive femoral neuropathy.
Bleeding into the iliacus fascial compartment from anticoagulation or trauma compressing the femoral nerve against the iliopectineal eminence, producing acute quadriceps weakness and anterior thigh numbness requiring urgent MRI and reversal of anticoagulation.
