Iliac Crest Posterior Periosteal Bursa

bursa periostei cristae iliacae posterioris

A periosteal bursa over the posterior iliac crest near the posterior superior iliac spine (PSIS), developing from repetitive pressure from posterior iliac crest bone graft harvest, direct trauma, or contact sports.

Region: Lower Back
Clinical Relevance

Clinical Notes

Posterior iliac crest bursitis is a well-recognised cause of donor site pain after iliac crest bone graft harvest for spinal fusion, hip surgery, or oral and maxillofacial procedures. The PSIS is directly under the skin with minimal soft tissue padding, making it susceptible to pressure bursitis. Padding and steroid injection address this bursa.
