Hepatic Capsule Bursa

bursa capsulae hepatis (Glissoni)

A potential bursal space between Glisson's capsule (the fibrous hepatic capsule) and the underlying hepatic parenchyma, which may accumulate fluid in subcapsular hepatic haematoma, liver abscess, or hepatic congestion. The capsule itself is relatively inelastic, making subcapsular collections painful from capsular stretching.

Clinical Notes

Subcapsular hepatic haematoma — occurring in trauma, HELLP syndrome of pregnancy, and hepatic artery aneurysm — expands in this subcapsular space, producing right upper quadrant pain from capsular stretch. CT characterises the collection and guides drainage. The Fitz-Hugh-Curtis syndrome (gonococcal perihepatitis) involves inflammation of the hepatic capsule producing violin string adhesions between the capsule and parietal peritoneum.

Blood accumulation between Glisson's capsule and the liver parenchyma from trauma, HELLP syndrome, or vascular injury producing right upper quadrant pain from capsular stretch, requiring CT characterisation and often conservative management.
