Gluteus Medius Bursa

The gluteus medius bursa is the primary bursa involved in greater trochanteric pain syndrome, lying directly between the gluteus medius tendon insertion and the trochanteric bone. Ultrasound demonstrates bursal distension in 50-60% of patients with GTPS, though the degree of bursal swelling correlates poorly with pain severity — the tendinopathy is the primary pain generator rather than the bursal inflammation.

Ultrasound-guided injection under the gluteus medius tendon at the superoposterior trochanteric facet targets the gluteus medius bursa specifically. The preferred injection approach is from inferior with the needle directed superiorly into the subtendinous bursal space. PRP injection into the bursa combined with high-volume image-guided injection (HVIGI) of the peritendinous space provides the strongest non-surgical evidence for GTPS.
