Gluteus Maximus Deep Bursa

A deep bursa between the gluteus maximus muscle and the posterior greater trochanter, separate from the superficial trochanteric bursa. It is compressed in external snapping hip syndrome when the posterior gluteus maximus margin snaps over the posterior greater trochanter.

Clinical Notes

External snapping hip (coxa saltans externa) is caused by the posterior gluteus maximus band snapping over the lateral greater trochanter, compressing this deep bursa. It produces a visible and audible lateral hip snap during hip flexion and extension. Dynamic ultrasound confirms the snapping band. Conservative management (stretching, injection) is first-line; surgical z-plasty of the gluteus maximus band addresses refractory cases.

Posterior gluteus maximus band snapping over the greater trochanter compressing the deep gluteal bursa, producing a lateral hip snap visible on dynamic ultrasound, managed by injection and gluteus maximus band release in refractory cases.
