Several bursae lie deep to the gluteus maximus, including the ischial bursa between the gluteus maximus and the ischial tuberosity and the gluteofemoral bursa between the gluteus maximus and the IT band. The ischial bursa (weaver's bottom) becomes inflamed from prolonged sitting on hard surfaces, producing ischial tuberosity pain that is reproduced by direct ischial pressure and resisted hip extension.
Reduces friction between the gluteus maximus and the underlying bony prominences during hip movement
Ischial bursitis (weaver's bottom) produces posterior buttock pain at the ischial tuberosity level reproduced by sitting and direct ischial palpation. It must be distinguished from proximal hamstring tendinopathy (also at the ischial tuberosity) by MRI showing the bursal fluid separate from the hamstring origin. Corticosteroid injection provides effective relief.
Ischial bursa inflammation from prolonged sitting producing deep buttock pain at the tuberosity level managed with padding, activity modification, and corticosteroid injection.
