The radial bursa of the wrist is the synovial sheath surrounding the flexor pollicis longus tendon as it passes through the carpal tunnel, extending from the wrist to the thumb IP joint. It is the palmar component of the FPL tendon sheath and the most lateral of the flexor tendon bursae.
FPL radial bursa infection produces a horseshoe abscess when the radial bursa communicates with the ulnar bursa through a pinhole connection. Stenosing tenosynovitis of FPL at the A1 pulley (trigger thumb) involves this bursa proximal to the pulley. De Quervain surgery must be distinguished from FPL pathology, which is more ulnar and proximal than the first extensor compartment.
