A small bursa at the fibular origin of flexor hallucis longus in the posterior lower leg, between the muscle and the posterior fibular shaft. It cushions the muscle against the bone during FHL contraction and ankle plantarflexion.
FHL origin bursitis is an uncommon cause of deep posterior lower leg pain in dancers and gymnasts, who place extreme demands on the FHL in forced plantarflexion. It is distinguished from FHL tendinopathy at the ankle by its proximal location at the fibular shaft. MRI confirms bursal fluid at the fibular FHL origin. Management involves activity modification and posterior compartment stretching.
