Flexor Digitorum Longus Ankle Bursa

bursa tendinis flexoris digitorum longi ad mallleolum

A bursa associated with the FDL tendon as it passes posterior to the medial malleolus within the tarsal tunnel, between the tendon sheath and the retinaculum. It supplements the FDL tendon sheath and reduces friction at the acute angulation behind the medial malleolus.

Clinical Notes

FDL ankle bursitis contributes to posteromedial ankle pain in tarsal tunnel syndrome and FDL tendinopathy. Ultrasound demonstrates peritendinous fluid specific to the FDL sheath at the medial malleolus level, distinguishing it from FHL and tibialis posterior pathology. FDL tenosynovitis is less common than tibialis posterior tenosynovitis at this level but produces similar symptoms.

Peritendinous bursal inflammation around the FDL at the medial malleolus producing posteromedial ankle pain with lesser toe flexion activities, managed with injection into the FDL tendon sheath.
