A synovial bursa surrounding the flexor carpi radialis tendon as it passes through its fibro-osseous tunnel at the wrist, between the ridge of the trapezium and the overlying transverse carpal ligament. It reduces friction as the FCR tendon changes direction to insert onto the second metacarpal base.
FCR wrist bursitis presents as palmar radial wrist pain reproduced by resisted wrist flexion and radial deviation. The fibro-osseous tunnel at the wrist is uniquely prone to tendinopathy and bursitis due to the acute angle the tendon takes around the trapezium ridge. Ultrasound demonstrates peritendinous fluid in this tunnel. Surgical release of the FCR tunnel is effective for refractory FCR tendinopathy and bursitis.
