Extensor Retinaculum Bursa of the Wrist

Small bursae between the extensor retinaculum and the underlying extensor tendon sheaths at the wrist, supplementing the synovial sheaths in reducing friction as the tendons pass beneath the retinaculum. They are most prominent in the first and sixth extensor compartments.

Clinical Notes

Extensor retinaculum bursitis contributes to dorsal wrist pain in de Quervain tenosynovitis (first compartment) and ECU tendinopathy (sixth compartment). Ultrasound demonstrates fluid between the retinaculum and the tendon sheath distinct from intrasheath fluid. Release of the retinaculum in de Quervain surgery simultaneously decompresses the associated bursa.

Bursal inflammation under the first extensor compartment retinaculum accompanying de Quervain tenosynovitis, contributing to the tender radial wrist swelling and pain with thumb motion.
