Extensor Carpi Ulnaris Forearm Bursa

A bursa between the ECU muscle belly and the underlying ulna in the distal forearm, distinct from the ECU subsheath bursa at the wrist. It reduces friction as the ECU glides over the ulnar dorsal surface during wrist extension and ulnar deviation.

Clinical Notes

ECU forearm bursitis contributes to dorsal forearm pain in the distal ulna territory, reproduced by resisted wrist extension-ulnar deviation. It is distinct from the ECU wrist-level subsheath pathology. Ultrasound identifies the bursa between the ECU belly and ulnar periosteum at the distal forearm level.
