A small bursa between the distal fibular periosteum and the overlying peroneal retinaculum, developing from repetitive pressure of the peroneal tendons against the lateral malleolus periosteum. It may become inflamed in peroneal tendon pathology.
Distal fibular periosteal bursitis contributes to lateral malleolus pain in peroneal tendinopathy and subluxation syndromes. Ultrasound identifies the bursa between the tendon sheath and the malleolus cortex. In ankle fracture fixation, periosteal elevation from the distal fibula is required for plate placement, and bursitis in this region may contribute to post-fixation lateral ankle discomfort.
