Deep Trochanteric Bursa

The deep trochanteric bursa (gluteus minimus bursa) lies between the gluteus minimus tendon and the anterior facet of the greater trochanter, the deepest of the peritrochanteric bursae. It is distinct from the larger subgluteus maximus bursa and from the subgluteus medius bursa that lies between the medius tendon and the lateral facet. The deep bursa is specifically associated with gluteus minimus tendinopathy affecting the anterior trochanteric facet.

MRI of the greater trochanter demonstrates three peritrochanteric bursae: the subgluteus maximus (the classically described trochanteric bursa, between the IT band/glut max and the posterior trochanter), the subgluteus medius (between glut medius and lateral facet), and the deep subgluteus minimus bursa (between glut minimus and anterior facet). Distinguishing which bursa is inflamed guides targeted injection — injection into the wrong bursal space produces poor results. The deep trochanteric bursa is most often involved in anterior hip pain syndromes and in anterior glut minimus tendinopathy.

Deep Trochanteric Bursa Injection for Anterior Hip Pain

Gluteus minimus tendinopathy involving the anterior greater trochanteric facet produces anterior lateral hip pain reproducible by hip flexion and internal rotation; ultrasound-guided injection targeting the deep trochanteric bursa between the gluteus minimus and the anterior trochanteric facet specifically treats this component of greater trochanteric pain syndrome.
