A bursa between the spinous processes of C5 and C6, the most mobile cervical motion segment. This level has the highest interspinous bursal incidence due to the greatest range of flexion-extension motion creating the most interspinous friction.
C5-C6 interspinous bursitis produces midline posterior neck pain specifically at this level, reproduced by neck extension and direct interspinous palpation. It may coexist with C5-C6 disc disease and facet arthropathy. Cervical interspinous injection at this level provides both diagnostic confirmation and relief. It is the most commonly symptomatic cervical interspinous bursa.
