A bursa between the spinous processes of C5 and C6, the most mobile cervical motion segment. This level has the highest interspinous bursal incidence due to the greatest range of flexion-extension motion creating the most interspinous friction.

Clinical Notes

C5-C6 interspinous bursitis produces midline posterior neck pain specifically at this level, reproduced by neck extension and direct interspinous palpation. It may coexist with C5-C6 disc disease and facet arthropathy. Cervical interspinous injection at this level provides both diagnostic confirmation and relief. It is the most commonly symptomatic cervical interspinous bursa.

Localised bursal inflammation between C5 and C6 spinous processes producing posterior neck pain reproduced by extension and direct palpation, often coexisting with disc and facet pathology at this level.
