Small synovial bursae associated with the atlanto-occipital joint capsule, facilitating head nodding motion between the occipital condyles and the lateral masses of C1. They are extensions of the atlanto-occipital joint cavity.
Atlanto-occipital bursal distension is seen in rheumatoid arthritis and inflammatory spondyloarthropathies, contributing to upper cervical instability and suboccipital pain. Imaging with MRI demonstrates enhanced synovial proliferation. Steroid injection into the atlanto-occipital joint targets the associated bursa.
Inflammatory synovial thickening and bursal distension at C0-C1, producing suboccipital pain and restricted nodding, associated with rheumatoid arthritis and requiring MRI assessment for instability.
