Atlanto-occipital bursal distension is seen in rheumatoid arthritis and inflammatory spondyloarthropathies, contributing to upper cervical instability and suboccipital pain. Imaging with MRI demonstrates enhanced synovial proliferation. Steroid injection into the atlanto-occipital joint targets the associated bursa.
