Trapezoid

os trapezoideum

The trapezoid is the smallest distal carpal bone, tightly wedged between the trapezium and capitate in the distal row and articulating with the second metacarpal — its position makes isolated fracture rare.

Clinical Notes

The trapezoid is the smallest carpal bone and the least commonly injured, protected by its central position locked between the index metacarpal, capitate, and trapezium. Trapezoid dislocations are rare injuries associated with severe wrist ligamentous disruption. The FCR tendon travels through a dedicated tunnel adjacent to the trapezoid and can become stenotic there, producing FCR tunnel syndrome with chronic radial wrist pain.
