Transverse Process

processus transversus vertebrae

The transverse processes are the lateral projections from each vertebra at the junction of the pedicle and lamina, providing attachment for the intertransverse and rotator muscles, the deep erector spinae components, and in the thoracic spine the rib tubercle articulations. Cervical transverse processes contain the foramina transversaria transmitting the vertebral arteries at C1 through C6.

Anatomical Data

Structure & Landmarks

Articulations
  • Thoracic ribs at the costotransverse joints
  • Adjacent transverse processes via the intertransverse ligaments
Landmarks
  • Thoracic transverse process tips, palpable laterally with deep pressure
  • Cervical transverse processes with their foramina transversaria
  • Lumbar transverse processes, larger and more horizontal than thoracic
OssificationTransverse processes ossify from the primary vertebral arch centres, with separate ossification centres in the lumbar region (sometimes confused with fractures) appearing in the first decade and fusing completely by the mid-twenties.
Clinical Relevance

Clinical Notes

Transverse process fractures from direct lateral lumbar blows or significant flexion-distraction injuries indicate significant force transmission. Multiple lumbar transverse process fractures (particularly L1 and L2) are associated with retroperitoneal haemorrhage from the lumbar vasculature and renal injuries from the force required to fracture these strong processes. Isolated single transverse process fractures are stable injuries managed conservatively with analgesics.

Pathology

Common Injuries & Conditions

Transverse Process Fracture

Usually from direct trauma or violent muscle pull, most often in the lumbar spine; these are stable but can signal abdominal or renal injury.
