The fibular crest is the anterior border of the fibular shaft, providing the anterior intermuscular septum attachment that separates the anterior compartment from the lateral (peroneal) compartment. In the distal fibula it becomes the anterior fibular margin that is palpable just lateral to the lateral malleolus.

Clinical Notes

The fibular crest is the landmark for the fasciotomy incision in the lateral compartment, where the crural fascia is incised anterior to the crest to avoid the superficial peroneal nerve. It is also the reference point for plate placement in fibular shaft ORIF. Periosteal reaction along the fibular crest is an early radiographic sign of fibular stress fracture.
