The deltoid tuberosity is the V-shaped roughened area on the anterolateral surface of the mid-humeral shaft, providing the insertion for the deltoid muscle. It marks the junction between the proximal and middle thirds of the humeral shaft and is the dominant bony landmark of the lateral upper arm.
The deltoid tuberosity is a key landmark in humeral shaft fracture classification, as fractures proximal to it differ in deformity pattern from those distal to it (proximal fragment abducted by deltoid, distal fragment adducted). Intramedullary nailing of humeral shaft fractures may damage the deltoid at the proximal entry point. Palpation of the tuberosity localises the mid-shaft in surgical approaches.
