The cuboid is the lateral column keystone, articulating with the calcaneus proximally, the fourth and fifth metatarsals distally, and the navicular and lateral cuneiform medially. The peroneus longus tendon grooves its plantar surface as it crosses the foot. The cuboid notch creates the tunnel for the peroneus longus crossover — a site of peroneus longus tendinopathy. Cuboid subluxation (cuboid syndrome) is common in ballet dancers and athletes.
Cuboid syndrome (subluxation of the cuboid from the calcaneus and fourth/fifth metatarsals) produces lateral midfoot pain reproduced by lateral cuboid compression. The cuboid whip manipulation — plantarflexing the foot and applying a thrust to the plantar cuboid — reduces the subluxation. Cuboid stress fractures produce lateral midfoot pain in runners.
